Juventus midfielder Kwadwo Asamoah will leave the club and new Chelsea manager Antonio Conte is hoping to reunite with the Ghanaian, Calciomercato.com has reported.
The 27-year-old has struggled to secure a regular place with the Scudetto champions this season after suffering a knee injury.
He was one of the most important players for Conte when the Italian was in charge of Juventus as he can play in a number of different positions.
According to more reports from Italy, the Blues are ready to pay £25million to secure the services of the player, who only played 13 games this season.
